You will need to bring the following information to your appointment:

    1.  Most recent 2-3 paycheck stubs.

    2.  Current shot records for each child being enrolled.

    3.  Well check statement from the doctor for each child being enrolled that states that child is in good health and may attend childcare (you can have this faxed to 806-741-0503.)

    4.  Name, address and telephone number of the child(rens) doctor.

    5.  Name, address and telephone number of at least 2 people, other than yourself who can, and will, pick the child(ren) up from the center if we are unable to reach you.

    6.  Food Stamp or TANF number if applicable.
